If you use ENVELOPES and need a year end tax statement for 2015, please EMAIL your request to sienastatements@gmail.com. This is NOT a website. Please include:

• Your full name(s)
• Your address
• Your envelope number

Please do not wait until the last minute. Email requests are done once a week through February, so if you plan to get a statement, don't delay.

If you use Parish Pay, you already should have received an email with your 2015 Annual Statement of donations attached. You may also obtain your statement by clicking on the Parish Pay logo, logging into your account and navigating to the "Reports" tab. From there, you can select the "Annual Statements" icon, and download the report as a PDF file.
